The community outreach service aims to achieve person centred outcomes through support in the community.

Complex support

Complex care experts offer support solutions in challenging and specialist situations.

Residential

The Elliots Hill residential care service supports people in a shared environment and is intended to be a stepping stone to a more independent setting such as supported living or community outreach.

Short breaks

Short breaks are temporary support for people who are normally supported by family or carers, giving them a break and allowing the supported person opportunites to meet new people and learn new skills.

Supported Living

Supported Living is a service that enables people to live in their own home instead of in residential care or with their family.
